The Intake Nurse provides knowledgeable, compassionate, and clinically sound nursing care through initial and ongoing patient assessments and care coordination for Residential, PHP/IOP, and/or LIR clients. This role is essential to ensuring a safe, thorough, and supportive admission process while maintaining accurate clinical documentation and collaboration across treatment teams. The Intake Nurse supports patients and families during a potentially stressful transition into care and ensures all medical, safety, and compliance requirements are met upon admission.

Responsibilities

• Conduct initial patient assessments to obtain medical history, current health status, and presenting concerns related to admission.
• Record and document vital signs, medications, allergies, and other relevant patient information in the electronic medical record (EMR).
• Collaborate with providers and interdisciplinary team members to support individualized care planning.
• Ensure patients are comfortable and have access to necessary medications, medical supplies, and personal belongings upon admission.
• Complete medication inventories for all medications brought in by clients and accurately enter information into the EMR.
• Perform nursing assessments including, but not limited to, body searches, urine drug screens, vital signs, and withdrawal assessments (COWS/CIWA).
