Intake Coordinator (Field Based)

Bluelake Homecare is hiring an Intake Coordinator to work with clients already in our pipeline and get them approved for care. Once a client enters intake, you will assist in: gathering documents, completing Medicaid paperwork, and traveling to client homes when a family needs help getting forms done right. You will also be present at the in-home MDHHS assessment that determines whether a client qualifies and how many hours of care they receive each week. What You Will Do Move clients through intake to start of careCollect and verify consents, physician paperwork, and Medicaid formsTravel to homes to help families complete documents in personAttend MDHHS in-home assessments and support the client and familyPrep families beforehand so they can clearly describe their daily needs What You Need Detail oriented and organizedStrong communicator, patient with seniors and familiesComfortable working independently in the fieldValid Michigan driver's license, reliable vehicle, current insuranceAble to pass a background check Nice to Have Experience with Michigan Medicaid, Home Help, MI Choice Waiver, home care, social services, or medical intake.
