Responsibilities

  • Complete the Eligibility Screening Disposition Form (Category 1) confirming a very low-income Veteran family’s eligibility for supportive services and documenting eligibility for SSVF homelessness prevention assistance.
  • Screen and verify eligibility requirements for SSVF homeless veteran’s assistance using federal and local public benefit program criteria.
  • Assist clients with crisis intervention and short-term housing stabilization, including access to emergency shelters/transitional housing and supportive services in both rural and urban communities.
  • Refer clients who do not qualify for SSVF to mainstream services or other housing stabilization programs in their service area.
  • Apply Presumptive eligibility criteria and document requirements (e.g., HINQ, VA ID Cards, SOS) during intake.
  • Track clients who are screened but not eligible for SSVF.
  • Implement the Housing First Model during client assessment.
  • Maintain intake logs, screening forms, and client databases with accurate personal information.
  • Enter and maintain data in the Homeless Management Information System (HMIS) ensuring accuracy and completeness.
  • Communicate the SSVF program process and required documentation to potential participants.
  • Participate in outreach activities: street/shelter outreach, coordinated-entry meetings, Stand Down, resource fairs, Week of Valor/Veterans Day events, and annual PIT count.
  • Demonstrate culturally appropriate engagement with Veterans and their families.
  • Participate in staff meetings, supervision, case consultation, and training; identify systemic barriers and collaborate with leadership on solutions.
  • Perform other duties as assigned by the SSVF Program Manager, Senior Management, and PQI committee.

Qualifications

  • Bachelor’s degree and less than five years of experience in the field; or five years of experience in the field and no degree; or a veteran with three years of related work experience.
  • Knowledge of customer service and proficiency with telephone systems and computers.
  • Strong organizational skills, self-starter mentality, and ability to work independently and as part of a team.
  • Excellent verbal and written communication skills; creative problem-solving; high degree of organization.
  • Personal automobile, valid driver’s license, liability insurance, and willingness to travel between counties served.
  • Ability to maintain confidentiality of all persons requesting assistance, services, and outcomes.
  • Demonstrated initiative and commitment to continuous process and relationship improvement.
  • Excellent time management with ability to manage multiple projects and meet deadlines.
  • Non-judgmental, empathetic, and respectful approach to serving homeless veterans.
  • Experience with HMIS is preferred but not required.
