About the Role
Coordinates and assists in the provision of health resources and services to clients.
Responsibilities
- Assists families in determining needed resources and services through a screening and triage process.
- Answers questions and concerns of families and provides service referrals and scheduling.
- Gathers referral and pre-admission information and completes all pre-admission data entry.
- Identifies emergent or complex cases and seeks clinical review prior to finalizing the referral.
- Collaborates with interdisciplinary teams to ensure efficient resource and scheduling services.
- Utilizes Electronic Medical Record and Scheduling software to assess the demand for services and the ability of current service capacities to meet the community’s needs.
- Maintains confidentiality of all clinical and administrative information consistent with agency policy.
Requirements
- Bachelor’s Degree in relevant field, or equivalent experience, required.
- Customer service or other relevant experience, required.
Skills
- Strong skills in customer service, process improvement, data entry, and advanced computer skills, required.
- Proficiency with EPIC, preferred.
Physical Requirements
- Occasionally: Bend/twist, climb stairs/ladder, flexing/extending of neck, lifting/carrying 0-10 lbs, pushing/pulling 0-25 lbs, reaching above shoulder, squat/kneel.
- Frequently: Standing, walking.
- Continuously: Audible speech, color vision, computer skills, decision making, depth perception, hand use (grasping, gripping, turning), hearing acuity, interpreting data, peripheral vision, problem solving, repetitive hand/arm use, seeing (far/near), sitting.
