Intake Coordinator

Overview

Good Shepherd Hospice is seeking a compassionate Hospice Intake Coordinator to serve in their Olathe and surrounding communities. The position involves assisting in gathering required patient information and coordinating enrollment of new patients into the hospice program. Hours are Monday-Friday, 8:00am-5:00pm.

Responsibilities

• Initial contact person for all new inquiries and referrals
• Responds promptly to any referral or inquiry
• Captures scheduling of intake personnel to follow up with referrals
• Provides guidance to hospice staff during the referral and enrollment process
• Informs the RN Case Manager of eligibility and enrollment into hospice program
• Coordinates all admission enrollment paperwork
• Obtains necessary patient information such as history and physicals, progress notes, lab results, nurse’s notes, and medication sheets
• Maintains regular communication with the referral source
• Reviews completed enrollment packets for accuracy and forwards for billing and filing
• Enters initial referral information into the computer
• Communicates with medical professionals, the hospice team, and others
• Coordinates completion of admission packets
• Maintains accurate, up-to-date records
• Demonstrates strong communication and customer service skills

Qualifications

• Hospice Intake experience required
• Valid driver’s license
• Demonstrated commitment to the hospice philosophy of care
• Ability to successfully pass background checks and drug screen
