Responsibilities
- Coordinate patient referrals and serve as a resource for referral sources
- Collaborate with the branch and sales teams to ensure adequate coordination and transition to services
- Apply and demonstrate knowledge of regulatory guidelines and governing processes
Requirements
- Must possess a high school diploma or equivalent
- Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics
- Must have demonstrated experience in customer service
- Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice
- Must have basic demonstrated technology skills, including operation of a mobile device
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
Qualifications
A graduate of an approved school of professional nursing and licensed in the state in which they currently practice is preferred; may be a registered nurse or licensed professional/vocational nurse.
Experience with intake responsibilities and working with a multi-location company is preferred.
