Intake Coordinator

Enhabit Home Health & Hospice, one of the nation's largest home-based care providers, is seeking a team member to coordinate patient referrals and serve as a resource for referral sources. Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources
• Collaborate with the branch and sales teams to ensure adequate coordination and transition to services
• Apply and demonstrate knowledge of regulatory guidelines and governing processes

Qualifications

• High school diploma or equivalent
• At least two years of experience in the health care industry
• One year experience in home health or pediatrics
• Demonstrated experience in customer service
• Demonstrated knowledge of Medicare guidelines that govern home health or hospice
• Basic demonstrated technology skills, including operation of a mobile device

Preferred Education and Experience

• Graduate of an approved school of professional nursing and licensed in the state in which they currently practice (RN or LPN/LVN preferred)
• Experience with intake responsibilities and working with a multi-location company preferred

Requirements

• Valid state driver license
• Automobile liability insurance as required by law
• Dependable transportation in good working condition
• Ability to safely drive an automobile in all types of weather conditions
