About the Role

This is a professional position providing intake informational support to individuals with special needs resulting from a developmental disability. The Human Services Program Analyst (Intake) is responsible for maintaining an intake caseload to determine APD eligibility. The role involves consulting with individuals, families, and others regarding APD eligibility requirements, services offered, Pre-Enrollment categories, and the application process.

Responsibilities

  • Complete the initial screening form and explain the intake process via phone, email, mail, or in-person visits.
  • Ensure information gathered for each intake is posted and/or emailed to the area supervisor/Lead.
  • Process all assigned APD application packets and mail them out after completing the screening form; document the application process within the system tracker.
  • Conduct quality reviews of documentation by obtaining copies of applicants' documents such as birth certificates, social security cards, Medicaid numbers, and collateral information to determine eligibility for APD services and potential transfer to Pre-Enrollment.
  • Update all progress notes for each applicant's file and generate the required Notice of APD Eligibility/Ineligibility in the APD electronic records system – iConnect.
  • Initiate and complete the short version support plan once eligibility approval has been determined; ensure the individual and/or their legal guardian completes and signs the support plan and the required initial Home Community Based Eligibility Worksheet.
  • Input, update, and accurately maintain all referral, applicant information, and forms in iConnect or other electronic systems utilized by APD.
  • Maintain and review high-risk individuals' case files (ages 3-5) to determine continuity of APD services by gathering additional collateral information after the consumer turns five years of age.
  • Perform other related duties or assignments as required.

Requirements

  • Valid Florida Driver License or other efficient means of transportation to travel for work purposes.
  • Experience working with developmental disabilities, mental illness, or the principles and practices of one of the allied health field principles or rehabilitative sciences.
  • Preference for fluent bilingual skills in Creole or Spanish (speaking and writing).
  • Ability to travel to surrounding areas, including group homes, foster homes, and family homes.
  • Ability to plan, organize, and coordinate work assignments.
  • Ability to interview applicants for public benefits assistance and services.
  • Ability to work under pressure, multi-task, and work independently.
  • Ability to establish and maintain effective working relationships with consumers, families, providers, and staff within and across regions.
  • Ability to prioritize and schedule work efficiently, determine work priorities, and ensure completion of work assignments.
  • Skilled in using Microsoft Office programs, applications, and web-based programs.

Pay

$34,760.00 - $36,000.00 annually | $1,336.92 - $1,384.61 bi-weekly

Additional Information

  • This position is not a telework position and requires reporting to the assigned APD office.
  • Applicants must complete all fields in the Candidate Profile, including work history with month and year.
