About the Role
Facilitate the transport of individuals and families to shelter. Conduct initial diversion/intake screening prior to contacting 211 to verify homelessness eligibility. Support the quarantine placement of COVID-positive individuals and families in DSS-contracted hotels.
Responsibilities
- Respond to all incoming calls from clients and/or providers
- Complete a diversion screening with walk-in clients prior to calling 211 to confirm homelessness eligibility
- Pick up clients and transport them to the appropriate shelter as determined by 211/DSS
- Support quarantine of COVID clients through proper documentation, transportation to quarantine, and providing food, hygiene items, and hotel rules
- Work with designated hotel agencies/shelters to provide a warm client hand-off
- Maintain and update transportation logs and documentation via shift notes
- Communicate with other shift Program Associates/Caseworkers/admin as needed
- Ensure adequate staffing prior to leaving a shift
- Observe all safety rules and regulations, including use of personal protective equipment
Requirements
- High School Diploma or G.E.D. with up to three (3) years of relevant experience or an equivalent combination of education and experience
- Experience working with the homeless population preferred
- Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities, which may not meet Federal accessibility standards
- Mobility sufficient to work in an office environment
- Manual dexterity sufficient to operate office equipment
- Valid N.Y.S. Driver’s License
- N.Y.S. Central Registry clearance
