Inpatient Clinical Pharmacist III

Position Overview

Delivers pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost-efficient medication therapy. Exercises sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. May also be responsible for performing specific tasks and/or orienting other staff to the department.

Experience providing clinical pharmacy services in an Emergency Department setting is preferred.

Responsibilities

• Provide patient-centered clinical pharmacy services, including medication information and education.
• Prepare and distribute medications ensuring safe, effective, and cost-efficient therapy.
• Exercise sound judgment and clinical skills to contribute to patient care plans.
• Assess patients competently and monitor care attentively.
• Communicate effectively with patients, families, and healthcare providers.
• Adhere to regulations, policies, and procedures for privacy and safety.
• Perform specific tasks and orient other staff as needed.
• Ensure the privacy of each patient’s Protected Health Information (PHI).
• Handle hazardous drugs in compliance with USP 800 guidelines and NIOSH standards.

Requirements

Education

• PharmD – Graduate of an accredited pharmacy school.

Certification & Licensure

• Registered Pharmacist (RPH) within 120 days.
• Advanced Cardiac Life Support (ACLS) if required by the entity/department.
• Pediatric Life Support (PALS) if required by the entity/department.

Experience

• 1 year of recent relevant experience.

Skills and Knowledge

• Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations, and laws and regulations.
• Understanding of different pharmacy practice settings, including narcotic delivery and procedures.
• Ability to identify and employ pharmaceutical and medical terms, abbreviations, and symbols commonly used in prescribing, dispensing, and record-keeping of medications.
• Basic working knowledge of legal requirements and accreditation standards including NABP, TJC, Title XXII, DHS, DEA, FDA, and USP.
• Written and verbal communication skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical individuals.
• Well-developed time management and organizational skills, including prioritizing assignments and working within standardized procedures.
• General knowledge of computer applications such as Microsoft Office Suite (Word, Excel, and Outlook), related pharmaceutical technology, EHR, and EPIC.
• Ability to work independently and as part of a team, managing multiple tasks in an environment with interruptions.
• Skills to identify, evaluate, and resolve standard problems by selecting appropriate solutions from established options.

Schedule

• Full Time (32 hours per week).
• Day/Evening/Night shift, 8-hour shifts.
• Variable days of the week, including every other weekend.

Pay

Pay Range is $87.25 to $122.15 per hour. The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
