Inpatient Clinical Pharmacist III

Position Overview

Delivers pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost efficient medication therapy. Exercises sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. May also be responsible for performing specific tasks and/or orient other staff to the department.

Responsibilities

• Provide patient-centered clinical service, medication information, and education.
• Prepare and distribute medications ensuring safe, effective, and cost-efficient therapy.
• Implement and contribute to the patient's plan of care based on diagnosis.
• Assess, monitor, and communicate effectively with patients, families, and healthcare providers.
• Adhere to local, state, and federal regulations, codes, policies, and procedures.
• Perform specific tasks and orient other staff to the department as needed.
• Ensure the privacy of each patient’s Protected Health Information (PHI).
• Handle hazardous drugs in accordance with USP 800 guidelines, including managing, storing, preparing, receiving, unpacking, transporting, disposing of, or administering such drugs.

Requirements

• Graduate of an accredited pharmacy school (PharmD).
• Registered Pharmacist (RPH) within 120 days of hire.
• Advanced Cardiac Life Support (ACLS) certification if required by the entity/department.
• 1 year of recent relevant experience.

Skills and Knowledge

• Knowledge of medical terminology, generic and trade pharmaceutical names, pharmaceutical calculations, and laws and regulations.
• Understanding of different pharmacy practice settings, including narcotic delivery and procedures.
• Ability to identify and employ pharmaceutical and medical terms, abbreviations, and symbols used in prescribing, dispensing, and record keeping.
• Basic working knowledge of legal requirements and accreditation standards (NABP, TJC, Title XXII, DHS, DEA, FDA, USP).
• Strong written and verbal communication skills to explain sensitive information clearly and professionally to diverse audiences, including non-medical individuals.
• Well-developed time management and organizational skills, with the ability to prioritize assignments and meet deadlines.
• Proficiency in computer applications such as Microsoft Office Suite (Word, Excel, Outlook), related pharmaceutical technology, EHR, and EPIC.
• Ability to work independently and as part of a team, handling multiple tasks in an environment with interruptions.
• Problem-solving skills to identify, evaluate, and resolve standard issues using established options.

Pay

Pay range is $87.25 to $122.15 per hour. The compensation range may vary based on geographic location, candidate experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.

Schedule

• Varied schedule (per diem/casual).
• Shift hours: 8.
• Days of the week: Variable.
• Weekend requirements: As needed.
• Weekly hours: 0 (per diem/casual).
