Inpatient Clinical Pharmacist III

This is a Limited Term Position, 26 weeks.

About the role

Delivers transitions of care services through coordination of medication reconciliation, medication access and coverage, and post-discharge calls while delivering patient education across care settings to ensure safe, accurate, and timely medication use at discharge. This role collaborates with providers and care teams to reduce medication-related errors, prevent readmissions, and support continuity of care. Delivers pharmaceutical care through the provision of patient-centered clinical service, medication information, education, medication preparation and distribution that ensures safe, effective, and cost-efficient medication therapy.

Exercises sound judgment and developed clinical skills to provide input to and implement the patient's plan of care based on the diagnosis in a timely manner. Gains confidence and cooperation from the patient, their family/support group, and other healthcare providers through competent patient assessment, attentive monitoring and care, and effective communication. Adheres to all local/state/federal regulations, codes, policies and procedures to ensure privacy and safety while delivering optimal patient care. May also be responsible for performing specific tasks and/or orienting other staff to the department.

Responsibilities

• Coordinate medication reconciliation, access, and coverage.
• Conduct post-discharge calls to deliver patient education.
• Collaborate with providers and care teams to reduce medication-related errors and prevent readmissions.
• Provide patient-centered clinical services, medication information, and education.
• Prepare and distribute medications ensuring safe, effective, and cost-efficient therapy.
• Implement the patient's plan of care based on diagnosis.
• Assess, monitor, and communicate effectively with patients, families, and healthcare providers.
• Adhere to all regulations, policies, and procedures for privacy and safety.
• Perform specific tasks and orient other staff as needed.

Requirements

• Graduate of an accredited pharmacy school (PharmD).
• Registered Pharmacist (RPH) within 120 days of hire.
• ACLS (Advanced Cardiac Life Support) if required by the entity/department.
• PALS (Pediatric Life Support) if required by the entity/department.
• 1 year of recent relevant experience.

Skills and Knowledge

• Knowledge of medical terminology, generic and trade pharmaceutical names, calculations, and laws.
• Understanding of different pharmacy practice settings, including narcotic delivery procedures.
• Ability to identify and employ pharmaceutical and medical terms, abbreviations, and symbols.
• Basic working knowledge of legal requirements and accreditation standards (NABP, TJC, Title XXII, DHS, DEA, FDA, USP).
• Strong written and verbal communication skills for diverse audiences, including non-medical individuals.
• Well-developed time management and organizational skills.
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook), related pharmaceutical technology, EHR, and EPIC.
• Ability to prioritize tasks, work independently, and as part of a team in an interrupt-driven environment.
• Problem-solving skills to evaluate and resolve standard issues using established solutions.
• Commitment to ensuring the privacy of each patient’s PHI.
• Handling of hazardous drugs in accordance with USP 800 guidelines.

Schedule

• Full Time, 40 hours per week.
• Shift: Days, 8-hour shifts.
• Days of the week: Monday - Friday.
• Weekend requirements: Rotating weekends.

Pay

Pay Range is $82.78 to $115.89 per hour. The compensation range may vary based on geographic location, candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs.
