About the role

Siskin Hospital for Physical Rehabilitation is seeking a dedicated Inpatient Case Manager to guide patients through their rehabilitation process and contribute to their recovery journey.

Key Responsibilities

  • Conduct initial patient evaluations and provide an overview of their rehabilitation program.
  • Develop individualized treatment plans with the goal of helping patients return home.
  • Monitor patient and family progress toward short- and long-term rehab goals, ensuring all necessary care and services are in place for discharge.
  • Cook up Durable Medical Equipment (DME), medical equipment, and follow-up services before discharge.
  • Educate patients and caregivers on available follow-up therapy services and make referrals as needed.
  • Advocate for patients, ensuring they are actively involved in their rehabilitation journey.
  • Ensure the collection of data for PAI documentation and contribute to the discharge planning process.

Qualifications

  • Education: Minimum of an Associate's Degree in healthcare or a related field (preferred)
  • Experience: At least 1 year of experience in healthcare, ideally in a hospital or rehabilitation setting (inpatient hospital experience preferred)
  • Skills: Strong communication and interpersonal skills to work effectively with patients, families, and healthcare teams; Knowledge of rehabilitation processes and discharge planning; Ability to manage multiple tasks and responsibilities in a fast-paced environment; A passion for patient care and improving the quality of life for those in recovery
