Inpatient Care Navigator (RN or SW), Per Diem Weekend Shift, Care Navigation

Shady Grove Medical Center seeks to hire an Inpatient Care Navigator for the Care Navigation Team who will embrace the mission to extend God’s care through the ministry of physical, mental, and spiritual healing. Responsibilities

• Completes comprehensive psychosocial assessment with focus on patient’s requirements as they transition to the next level of care
• Collaborates with physicians, nurses, and other disciplines involved with care of the patient to foster a coordinated approach to discharge planning
• Identifies and navigates patient testing and treatment to reduce barriers to patient discharge and prevent delays in patient care; communicates barriers to leadership for resolution and trending
• Communicates with Utilization Review staff on any denials, issues or barriers to discharge
• Identifies services and resources available in the community and assists with patient connection to these services
• Participates in Interdisciplinary Rounds and other patient care conferences
• Documents assessments and interventions according to departmental standards
• Participates in process improvement activities

Qualifications

Social Worker track

• Bachelor of Social Work (BSW) or Master of Social Work (MSW)
• Minimum of 3 years of experience in diverse clinical setting
• Prior case management or discharge planning experience preferred
• Prior experience with Cerner EMR preferred
• LBSW, LMSW, or LCSW licensure
• Active American Heart Association Basic Life Support (BLS) certification required

Registered Nurse track

• Graduate of an accredited school of nursing with a minimum associate of science degree (bachelor’s degree preferred)
• Minimum of 1‑3 years clinical experience required
• Licensed as Registered Nurse
• Preferably prior case management or discharge planning experience
• Experience with Cerner EMR and All Scripts preferred
• Active American Heart Association Basic Life Support (BLS) certification required

Pay

• Per diem position: $48/hour for RN
• Per diem position: $45/hour for Social Worker

Schedule

• Per diem position
• Hours per week: 8 hrs/week
• Typical daily schedule: Every other weekend, minimum requirement of 3 shifts per month
• Shift type: Days
• Weekend requirements: as listed
• Holiday requirements: Follow company holiday calendar
• Shift differential eligible: Yes
