Inpatient Advance Pharmacy Technician (Medication Histories) - 40 hrs/wk - Macomb

About the role

The Inpatient Advance Pharmacy Technician (Medication Histories) is primarily assigned to the Emergency Department. Their main responsibility is to review and update patients' medication lists within the electronic medical record to ensure accuracy for individuals being admitted to the hospital. This involves reviewing office visit documentation, recent hospital discharge summaries, and pharmacy records, as well as conducting interviews with patients and their family members to confirm all current medications.

Responsibilities

• Thoroughly review office visit documentation, recent hospital discharge summaries, and pharmacy records to update medication lists in the electronic medical record.
• Conduct interviews with patients and their family members to confirm all current medications.
• Verify the names, dosages, and administration instructions of all medications, including prescription drugs, over-the-counter supplements, sample medications provided by physicians, and medications taken on an as-needed basis.
• Contact pharmacies or healthcare providers to obtain clarification or additional details when discrepancies arise or information is incomplete.
• Update the medication list in the electronic medical record, document findings in a progress note, and inform the provider that the medication history has been accurately updated.

Requirements

• A high school diploma or GED
• A valid Pharmacy Technician license from the State of Michigan
• Minimum one year of experience as a Hospital Pharmacy Technician or Retail Pharmacy Technician
• Ability to accurately maintain records and perform calculations
• Ability to safely handle and manipulate cytotoxic and other special-handling medications, when applicable
• Ability to work aseptically in a vertical flow hood environment, when required
• Ability to maintain Investigational Drug Research Records and validate doses using investigational drug protocols
• Ability to lift, carry, and stock pharmacy inventory and transport medications to nursing units
• Ability to use a computer keyboard and type approximately 25 WPM
• Ability to read, interpret, and apply literature related to medications requiring special handling
• Ability to assist in developing or modifying departmental protocols under pharmacist supervision

Qualifications
