Inpatient Admissions RN/Hospice

Key Reponsibilities

  • Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily in-person collaboration and communication.
  • Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
  • Conduct patient assessments and coordinate the admission process. Partner with Admissions, Clinical, and Care Teams for smooth transitions.
  • Advocate for patients and families during goals-of-care conversations.
  • Participate in interdisciplinary planning, training, and outreach.
  • Uphold ethics, quality, and continuous learning.
  • Meet expectations for response times, referral coordination, and communication.

Qualifications

  • One year of RN experience with knowledge of home health regulations, Medicare requirements, and care-plan development.
  • Medical sales or clinical outreach experience preferred.
  • Strong communication and relationship-building skills.
  • Able to work independently in a hospital environment.
  • Passion for serving with dignity and empathy.
  • Active RN license.
  • Valid driver’s license and insured vehicle.
