Role Details
- Schedule: Monday through Friday between 8:30am and 5:00pm Eastern Standard Time
- Location: LUX Infusion Pharmacy seeks a Full-Time Infusion Nurse to see patients in their homes in Syracuse, NY. Work location is subject to change based on business needs.
- On-Call Requirement: Participation in the on-call rotation is required. It is anticipated that an incumbent in this role would be on-call approximately once every eight (8) weeks, covering two (2) weekdays per month and one (1) weekend.
Responsibilities
- Provide direct home infusion nursing care by inserting, monitoring, and removing IVs and administering prescribed medications, blood products, and nutrition
- Maintain venous access, including peripheral IVs and central lines (PICC, port, midline)
- Teach patients and caregivers how to safely perform and monitor home infusion therapy
- Collaborate closely with physicians, nursing coordinators, directors of nursing, agencies, and internal care teams
- Coordinate start-of-care and ongoing visits, confirm schedules, and communicate updates across care partners
- Complete, review, and submit nursing notes, therapy administration reports, care plans, and progress documentation
- Communicate patient status changes, issues, or reportable events promptly to referring providers and internal teams
- Maintain compliance with all federal and state regulations, privacy requirements, and ethical standards
- Participate in staffing coordination and scheduling while maintaining confidentiality and professionalism
- Drive within the state to patient residences and assigned care locations
- Bring reliability, strong clinical judgment, and attention to detail to independent work in home settings
Qualifications
- Deep care about patient outcomes and the home-based care experience
- A current RN license in New York and at least two years of RN experience
- IV certification and experience with central line care and management
- Strong medical-surgical, phlebotomy, and venous access knowledge
- Confidence working independently while collaborating with multidisciplinary teams
- Excellent assessment, documentation, organizational, and multitasking skills
- Clear, compassionate communication with patients, caregivers, and providers
- Comfort managing high-acuity or stressful situations in the home environment
- Willingness to travel locally to patient residences and healthcare facilities
- A commitment to accountability, service excellence, and patient-first care
