About the role
This position will be primarily working in the homes of the people we support. Locations could include Washington County (Oakdale, Woodbury, North St. Paul) and Saint Paul. The ideal candidate for this role will be someone who is available to work Monday - Friday between the hours of 2pm and 8pm.
Responsibilities
- Assist adults with disabilities maintain independence and improve daily living skills.
- Absorb people to engage in meaningful activities both at home and in the community.
- Provide support to people in their homes focusing on:
- Community participation, health, safety, and wellness,
- Household management, and adaptive skills.
Assist with community participation
- Safely get in and around the community,
- Use and access community resources,
- Develop and support an informal support system and network,
- Enhance interpersonal communication skills,
- Plan and organize recreational and social activities,
- Build skills to meet transportation needs.
Assist with health, safety, and wellness
- Collaborate with the person to arrange appointments,
- Support, supervise, train or instruct to complete self-care,
- Support the person to achieve their health, safety, and wellness goals.
Assist with household management
- Guide and supervise to complete household care and maintenance tasks,
- Acquire and maintain household safety knowledge and skills,
- Guide and support with cooking, meal planning, healthy lifestyle skills, household chores, and personal needs purchasing.
Assist with adaptive skills
- Prevent crises and develop problem-solving skills,
- Support sensory and motor development,
- Support and train to increase positive behavior.
Maintain up-to-date records and documentation
- Accurately read and implement Coordinated Service and Support Plan/Addendums, medication instructions etc.,
- Accurately record observations including Accident/Incident Reports, Seizure Reports, and observations related to progress in meeting individualized plans,
- Accurately complete medication administration records and other paperwork related to Direct Support Professional duties.
Requirements
- Relate well with people with disabilities,
- Be qualified for employment in a 245d licensed program as determined by the MN Department of Human Services,
- Pass other background checks as required by MSS and/or state and federal law,
- Secure and maintain current certification in First Aid, CPR and Medication Certification,
- Have and maintain a valid driver's license and meet MSS standards for providing transportation services,
- May be responsible for transporting persons served in their personal vehicle as needed.
Individualized home support employees
- Must obtain a Medical Examiner Certificate if driving an agency vehicle,
- Must have a clean driving record and be insurable under the conditions set by our insurance carrier,
- Must provide proof of auto insurance upon hire.

- Starting wage for this position is $19.25/hour,
- Employees working 20 or more hours per week are eligible for company paid life insurance and have the opportunity to purchase medical coverage at a pro-rated cost,
