Individual Needs Assessor / JCOA

About the role

The Individual Needs Assessor at JCOA Main works in the Care Management Department and is responsible for assessing seniors age 60 and older to determine their eligibility for Jefferson Council on Aging services.

Responsibilities

• Completing Louisiana Independent Living Assessments and/or NFCSP Assessments, including initial in-home assessments after referral and annual reassessments.
• Calling participants, meeting with seniors and/or families in their homes, collecting signatures, entering information into JCOA's database, and submitting completed assessments every two weeks.
• Attending trainings or meetings as needed.

Requirements

• A 2-year degree or four years of relevant experience.
• Strong communication skills.
• Confidentiality.
• Attention to detail.
• The ability to work independently.
• Comfort interacting with elderly individuals from diverse backgrounds.

Qualifications

• No supervisory responsibilities.
• Non-exempt status.
• Pays $12.00 per hour.
