In-Home Traveler Nurse Practitioner or Physician Assistant (Full Time) - VA

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations.

Responsibilities

• Bring your heart into every visit, joining a national network of purpose-driven clinicians dedicated to improving health outcomes
• Part of our efforts to visit millions of people nationwide with the goal of providing connections to the right care for people’s unique needs
• Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role
• Flexible to travel locally and within licensed states
• Conduct virtual visits as needed
• Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)

Qualifications

• Holding multiple state licenses will be required, as dictated by business needs
• Able to obtain and maintain multi-state licenses in coverage area, as required
• Able to cover all counties within state and flex to other areas of business need

Benefits

The typical pay range for this role is: $87,035.00 - $187,460.00
