In-Home Services CNA

Responsibilities

• Providing routine care and assisting participants/clients with activities of daily living, restorative and supportive care.
• Physical attendance at an InnovAge facility and/or participant home is essential.
• Follows plan of care and assists with ADLs such as bathing, dressing, grooming, hair care, mouth care, eating, hand and toileting as assigned or needed.
• Documents care provided in accordance with established policies, guidelines, and applicable regulations.
• Carries out, reports, and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.
• Participates in care planning through cooperation with multidisciplinary teams.
• Monitors participants’ health status by taking vital signs and weights (monthly).
• Monitors appetite, functional ability changes, and skin condition while assisting with toileting.
• Assists participants courteously with meals and provides feeding assistance as necessary.
• Assists with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, filling oxygen, and providing other aides as assigned or needed.
• Reports pertinent health information (vital signs, weights, skin condition, appetite, functional ability changes) to clinical supervisor in a timely manner.
• Reports any variance, accident, or unusual occurrence to appropriate personnel.
• Attends in-service and continuing education programs as scheduled and necessary.
• Performs nursing assistant services to clients in their place of residence.
• Attends position-related in-services.
• Attends all mandatory in-service programs (minimum 12 hours/year).
• Participates in QM activities as requested.
• Completes appropriate visit records timely per agency policy.
• Performs simple procedures as an extension of therapy or nursing services as assigned.
• Performs household services essential to health care in the home as assigned.
• Does not accept assignments for patients with special needs without appropriate training.
• Additional work functions and duties may be assigned.

Requirements

• Current and valid state-issued Certified Nursing Assistant (CNA) license.
• Current First Aid and BLS certifications required prior to hire (acceptable vendors: American Heart Association and/or American Red Cross).
• Personal transportation with current state-issued driver’s license, good driving record, and auto insurance as required by law.
• Minimum of one year experience working with the frail or elderly.
• High School Diploma/GED or relevant work experience in lieu of education.
• Preferred: Bilingual.

Pay

Posted pay range: $20.67 - $24.81 per hour. Pay may vary based on job-related factors such as work location, experience, knowledge, skills, education, certifications, training, and internal equity.
