In-Home Services CNA
Responsibilities
- Provides routine care and assists participants or clients with activities of daily living, restorative and supportive care.
- Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties.
- Documents care provided in accordance with established policies, guidelines and applicable regulations.
- Carefully monitors participants’ health status by taking vital signs and weights (monthly).
- Makes observations about participants’ appetite, skin condition, and changes in functional ability.
- Reports any variance, accident or unusual occurrence to the appropriate personnel.
- Attends in-service and continuing education programs as scheduled and necessary.
- Performs nursing assistant services to clients in their place of residence.
- Completes appropriate visit records in a timely manner as per Agency policy.
- Performs simple procedures as an extension of the therapy or nursing services as assigned.
- Performs household services essential to health care in the home as assigned.
- Does not accept assignments for a patient with special needs for which he/she has not received appropriate training.
Requirements
- Current and valid state issued Certified Nursing Assistant (CNA) license.
- Current First Aid and BLS certifications are required prior to hire.
- Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
- Require personal transportation, current state issued driver’s license, good driving record and auto insurance as required by law.
- A minimum of one year experience working the frail or elderly.
- A High School Diploma / GED or relevant work experience in lieu of education.
