In-Home Services CNA

Responsibilities

• Provides routine care and assists participants or clients with activities of daily living, restorative and supportive care.
• Physical attendance at an InnovAge facility and/or an InnovAge participant home is essential to performing the job duties.
• Documents care provided in accordance with established policies, guidelines and applicable regulations.
• Carefully monitors participants’ health status by taking vital signs and weights (monthly).
• Makes observations about participants’ appetite, skin condition, and changes in functional ability and reports pertinent information to clinical supervisor in a timely manner.
• Reports any variance, accident or unusual occurrence to the appropriate personnel.
• Attends in-service and continuing education programs as scheduled and necessary.
• Performs nursing assistant services to clients in their place of residence.
• Completes appropriate visit records in a timely manner as per Agency policy.
• Performs simple procedures as an extension of the therapy or nursing services as assigned.
• Performs household services essential to health care in the home as assigned.
• Does not accept assignments for a patient with special needs for which he/she has not received appropriate training.

Requirements

• Current and valid state issued Certified Nursing Assistant (CNA) license.
• Current First Aid and BLS certifications are required prior to hire. Acceptable vendors for certifications are from either American Heart Association and/or American Red Cross.
• Requires personal transportation, current state issued driver’s license, good driving record and auto insurance as required by law.
• A minimum of one year experience working with the frail or elderly.
• A High School Diploma / GED or relevant work experience in lieu of education.

Qualifications

• Bi-lingual preferred.

Skills

• Excellent interpersonal and communication skills.
• Ability to work independently and as part of a team.
• Strong organizational and time management skills.
• Knowledge of basic first aid and CPR.
