In-Home Services CNA

Responsibilities

• Provide routine care and assist participants or clients with activities of daily living, restorative, and supportive care.
• Follow the plan of care and assist participants with activities such as bathing, dressing, grooming, hair care, mouth care, eating, hand hygiene, and toileting as assigned or needed.
• Document care provided in accordance with established policies, guidelines, and applicable regulations.
• Carry out, report, and document care given in an effective, timely manner as observed by the RN and through periodic record reviews.
• Participate in the care planning of participants through cooperation with multidisciplinary teams.
• Monitor participants’ health status by taking vital signs and weights (monthly).
• Monitor appetite, changes in functional ability, and skin condition while assisting with toileting.
• Assist participants courteously with meals and provide feeding assistance as necessary.
• Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, filling oxygen, and providing other aids as assigned or needed.
• Report pertinent health status information (e.g., vital signs, weights, skin condition, appetite, functional ability changes) to the clinical supervisor in a timely manner.
• Report any variance, accident, or unusual occurrence to the appropriate personnel.
• Attend in-service and continuing education programs as scheduled and necessary.
• Perform nursing assistant services to clients in their place of residence.
• Attend position-related in-services and all mandatory in-service programs (minimum 12 hours/year).
• Participate in Quality Management (QM) activities as requested.
• Complete appropriate visit records in a timely manner as per agency policy.
• Perform simple procedures as an extension of therapy or nursing services as assigned.
• Perform household services essential to healthcare in the home as assigned.
• Do not accept assignments for patients with special needs for which you have not received appropriate training.
• Additional work functions and duties may be assigned.

Requirements

• Current and valid state-issued Certified Nursing Assistant (CNA) license.
• Current First Aid and BLS certifications (acceptable vendors: American Heart Association and/or American Red Cross).
• Personal transportation, current state-issued driver’s license, good driving record, and auto insurance as required by law.
• A minimum of one year of experience working with the frail or elderly.
• High School Diploma/GED or relevant work experience in lieu of education.

Preferred

• Bilingual.

Pay

Posted pay range: $20.67 - $27.00 per hour.
