In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations. You’ll engage with individuals in their homes, bridging gaps in care through flexible, comprehensive, and personalized health assessments. The role focuses on building trusted relationships to improve health outcomes without altering existing treatment regimens or prescribing medications.
Responsibilities
- Conduct 45-minute in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests (e.g., diabetic eye exams, spirometry) as ordered by the health plan.
- Use an iPad with an intuitive clinical workflow for each evaluation.
- Travel locally and within licensed states to perform in-home visits.
- Conduct virtual visits as needed.
- Provide Diagnostic & Preventive Services (e.g., spirometry, diabetic eye exams, bone density screening).
- Work within a national network of clinicians dedicated to improving health outcomes.
Work Environment
As an in-home health clinician, your primary workplace will be the private residences of members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.
Qualifications
- Active, unrestricted Nurse Practitioner or Physician Assistant license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
- Able to cover the following primary locations: Oneida/Syracuse, NY.

Pay
The typical pay range for this role is $46.03 - $99.14 per hour, based on factors such as experience, education, and geography. Pay is structured per completed In-Home Health Evaluation, with additional reimbursement opportunities for Diagnostic and Preventive Services (DPS) as ordered.
Schedule
This is a part-time position with an anticipated weekly commitment of 29.5 hours.
