In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations. You’ll engage with individuals in their homes, bridging gaps in care through flexible, comprehensive, and personalized health assessments. Visits last approximately 45 minutes and include a medication and medical history review, a physical evaluation, and—if ordered by the person’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. Using an iPad and an intuitive clinical workflow, you’ll focus on building trusted relationships without prescribing medicine, ordering lab tests, or altering existing treatment regimens.
A Note on Our Work Environment: Your primary workplace will be the private residences of our members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.
Responsibilities
- Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical exams.
- Perform diagnostic and preventive services (e.g., spirometry, diabetic eye exams, bone density screenings) as indicated.
- Travel locally and within licensed states to member homes, with flexibility for virtual visits as needed.
- Use an iPad and clinical workflow to document evaluations without administrative burdens.
- Provide education and guidance to members, addressing their health questions and concerns.
- Collaborate with a national network of clinicians dedicated to improving health outcomes.
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
- Able to cover the Saint Cloud, MN area and surrounding locations.

Pay
The typical pay range for this role is $41.85 - $90.13 per hour, depending on experience, education, geography, and other relevant factors.
Schedule
- Part-time, with an anticipated 40 hours per week.
