In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
As a Part Time Clinician (Nurse Practitioner or Physician Assistant) with Signify Health, part of CVS Health, you will provide In-Home Health Evaluations, engaging with individuals in the comfort of their own homes to help bridge gaps in care. This role focuses on building trusted relationships to improve health outcomes through flexible, comprehensive, and personalized health evaluations.
The visit, which lasts about 45 minutes, includes a one-on-one connection to answer health questions, a review of medication and medical history, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests such as diabetic eye exams or spirometry. You will use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter current treatment regimens.
A Note on Our Work Environment: Your primary workplace will be the private residences of health plan members. These environments are diverse and may present varying conditions, including household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in such settings.
Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests as needed.
- Engage with health plan members in their homes to build trusted relationships and improve health outcomes.
- Travel locally and within licensed states to perform evaluations, with flexibility for virtual visits as needed.
- Provide Signify Health services, including Diagnostic & Preventive Services products (e.g., spirometry, diabetic eye exams, bone density screening).
- Use an iPad and clinical workflow to document evaluations without administrative responsibilities.
- Educate members about their health and answer questions to help them better understand their care.
Qualifications
- Active, unrestricted license(s) as a Nurse Practitioner or Physician Assistant in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).

Pay
The typical pay range for this role is $41.85 - $90.13 per hour. The actual base salary offer will depend on factors such as experience, education, geography, and other relevant considerations.
Schedule
- Part-time position with an anticipated weekly commitment of 29.5 hours.
