In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations. You’ll engage with individuals in their homes, reviewing medical history, performing physical evaluations, and conducting diagnostic tests as needed. Visits last about 45 minutes and focus on building trusted relationships to improve health outcomes.
The role involves using an iPad with an intuitive clinical workflow. You will not prescribe medicine, order lab tests, or alter treatment regimens. Clinicians travel locally within licensed states and may conduct virtual visits as needed.
A Note on Our Work Environment: Your primary workplace will be the private residences of health plan members. These environments vary and may include pets, secondhand smoke, or other airborne irritants. Candidates must be comfortable and safe performing duties in diverse home settings.
Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the health plan.
- Build trusted relationships with members to improve health outcomes and address individual health needs.
- Travel locally and within licensed states to perform evaluations.
- Conduct virtual visits as needed.
- Provide Signify Health services, including Diagnostic & Preventive Services (e.g., spirometry, diabetic eye exams, bone density screenings).
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).

Pay
The typical pay range for this role is $41.85 - $90.13 per hour. The actual base salary offer will depend on factors such as experience, education, and geography.
Schedule
- Part-time: 29.5 hours per week.
