In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations. You’ll engage with individuals in their homes, helping bridge gaps in care through personalized health assessments. The 45-minute visit includes a medication and medical history review, a physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the member’s health plan. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter treatment regimens.
Responsibilities
- Conduct in-home health evaluations, building trusted relationships to improve health outcomes.
- Perform medication reviews, physical evaluations, and diagnostic tests as needed (e.g., spirometry, diabetic eye exams, bone density screenings).
- Travel locally and within licensed states to visit health plan members in their homes.
- Conduct virtual visits as required.
- Provide education and guidance to help individuals better understand and manage their health.
Work Environment
As an in-home clinician, your primary workplace is the private residence of health plan members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.
Requirements
- Active, unrestricted NP or PA license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
Schedule
- Part-time: 29.5 hours per week.
Pay
The typical pay range for this role is $37.66 - $81.11 per hour, depending on experience, education, geography, and other relevant factors.
