In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations. You’ll engage with individuals in their homes, bridging gaps in care through flexible, comprehensive, and personalized health assessments. The 45-minute visit includes a medication and medical history review, a physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the member’s health plan. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter treatment regimens.
A Note on Our Work Environment: Your primary workplace will be the private residences of health plan members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.
Responsibilities
- Conduct in-home health evaluations, building trusted relationships to improve health outcomes.
- Perform medication and medical history reviews, physical evaluations, and diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screenings) as indicated.
- Travel locally and within licensed states as needed.
- Conduct virtual visits when required.
- Provide all Signify Health services, including Diagnostic & Preventive Services products.

Requirements
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).

Pay
The typical pay range for this role is $41.85 - $90.13 per hour. The actual base hourly rate will depend on factors such as experience, education, and geography.
Schedule
- Part-time: 29.5 anticipated weekly hours.
