In-Home Nurse Practitioner or Physician Assistant Part Time
About the Role
Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations. This role focuses on building trusted relationships, performing comprehensive health assessments, and bridging gaps in care—all within the comfort of members' homes.
The 45-minute visit includes a medication and medical history review, a physical evaluation, and optional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the member’s health plan. You will use an iPad with an intuitive clinical workflow and are not required to prescribe medication, order lab tests, or alter existing treatment plans.
A Note on Our Work Environment: Your primary workplace will be the private residences of our members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.
Responsibilities
- Conduct in-home health evaluations, including medication reviews, physical assessments, and diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screenings).
- Build trusted relationships with members to improve health outcomes and address individual needs.
- Travel locally and within licensed states as needed, with flexibility for virtual visits.
- Provide all Signify Health services, including Diagnostic & Preventive Services products.
- Engage with a national network of clinicians dedicated to improving community health.
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
- Able to cover the Saint Cloud, MN area and surrounding locations.

Pay
The typical pay range for this role is $41.85 - $90.13 per hour. The actual base hourly rate will depend on factors such as experience, education, and geography.
Schedule
- Part-time, with an anticipated 40 hours per week.
