In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Evaluations. You’ll engage with individuals in their homes, bridging gaps in care through flexible, comprehensive, and personalized health assessments. Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and—if ordered by the person’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. Using an iPad and an intuitive clinical workflow, you’ll focus on building trusted relationships without prescribing medicine, ordering lab tests, or altering existing treatment regimens.
Responsibilities
- Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical exams.
- Perform diagnostic and preventive services (e.g., spirometry, diabetic eye exams, bone density screenings) as indicated.
- Travel locally and within licensed states to visit health plan members in their homes.
- Conduct virtual visits as needed.
- Use an iPad and clinical workflow tools to document evaluations.
- Build trusted relationships to improve health outcomes and connect individuals with appropriate care.
Work Environment
As an in-home clinician, your primary workplace is the private residence of health plan members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.
Requirements
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).

Pay
The typical pay range for this role is $37.66 - $81.11 per hour. The actual base salary offer will depend on factors such as experience, education, and geography.
Schedule
- Part-time: 29.5 hours per week.
