In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
Signify Health, part of CVS Health®, is seeking a Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. You’ll engage with people in the comfort of their own homes, helping bridge gaps in care by building trusted relationships and delivering flexible, comprehensive, and personalized health evaluations.
The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests (e.g., diabetic eye exams, spirometry). You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter people’s current treatment regimen.
A Note on Our Work Environment: As an in-home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present a variety of conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform their duties in these settings.
Responsibilities
- Conduct in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests (e.g., spirometry, diabetic eye exams) as ordered by health plans.
- Build trusted relationships with health plan members to improve health outcomes.
- Travel locally and within licensed states to perform in-home visits.
- Conduct virtual visits as needed.
- Provide all Signify Health services, including Diagnostic & Preventive Services products (e.g., spirometry, DEE).
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license(s) in coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
- Able to cover the following primary locations: Houghton (Counties: Gogebic, Houghton, Keweenaw, Ontonagon).

Pay
The typical pay range for this role is $41.85 - $90.13 per hour. The actual base salary offer will depend on factors including experience, education, geography, and other relevant considerations.
Schedule
- Part-time: 29.5 hours per week.
