In-Home Nurse Practitioner or Physician Assistant Part Time

About the role

Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to conduct in-home health evaluations. You’ll engage with individuals in their homes, review medical histories, perform physical evaluations, and administer diagnostic tests as needed. The role focuses on building trusted relationships to improve health outcomes without altering treatment regimens or prescribing medications.

Responsibilities

• Conduct 45-minute in-home health evaluations, including medication and medical history reviews, physical evaluations, and diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the health plan.
• Use an iPad with an intuitive clinical workflow for evaluations.
• Travel locally and within licensed states to perform in-home visits.
• Conduct virtual visits as needed.
• Provide Signify Health services, including Diagnostic & Preventive Services (e.g., spirometry, diabetic eye exams, bone density screening).
• Work in diverse home environments, which may include pets, secondhand smoke, or other airborne irritants.

Requirements

• Active, unrestricted NP or PA license(s) in the coverage area(s).
• Board certification (required).
• Multi-state licenses (preferred).
• Able to cover Worcester and the surrounding area.

Pay

The typical pay range for this role is $46.03 - $99.14 per hour, depending on experience, education, geography, and other relevant factors.

Schedule

• Part-time, with an anticipated 29.5 hours per week.
