In-Home Nurse Practitioner or Physician Assistant Part Time
About the role
As a Part-time Clinician (Nurse Practitioner or Physician Assistant) with Signify Health, part of CVS Health, you’ll provide In-Home Health Evaluations in the comfort of members’ homes. This role focuses on building trusted relationships, conducting comprehensive health evaluations, and helping bridge gaps in care. Each visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and—if ordered by the member’s health plan—additional diagnostic tests such as diabetic eye exams or spirometry. You’ll use an iPad with an intuitive clinical workflow and are not required to prescribe medicine, order lab tests, or alter treatment regimens.
Responsibilities
- Conduct in-home health evaluations, including medication reviews, medical history assessments, and physical evaluations.
- Perform diagnostic and preventive services (e.g., spirometry, diabetic eye exams) as indicated by health plans.
- Travel locally and within licensed states to visit members in their homes.
- Conduct virtual visits as needed.
- Provide education and guidance to members to improve their understanding of health and care options.
- Collaborate with a national network of clinicians dedicated to improving health outcomes.
Work Environment
Your primary workplace will be the private residences of health plan members. These environments vary and may include household pets, secondhand smoke, or other airborne allergens. Candidates must be comfortable and safe performing duties in diverse home settings.
Qualifications
- Active, unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the coverage area(s).
- Board certification (required).
- Multi-state licenses (preferred).
- Able to cover the Plattsburgh, NY area and surrounding locations.
Schedule
- Part-time: 29.5 hours per week.
Pay
The typical pay range for this role is $46.03 - $99.14 per hour, depending on experience, education, geography, and other relevant factors.
