In Home Healthcare RN - Day Shifts

About the role

We are seeking a Registered Nurse (RN) to provide skilled nursing care in patients' homes, primarily for preteen children with high acuity needs. The role involves delivering and coordinating quality patient care according to physician orders, assessing patient conditions, documenting activities, and developing care plans.

Responsibilities

The RN must recognize changes in patient needs, implement appropriate interventions, and educate patients and families on disease management and self-care. Responsibilities include adhering to infection control standards, maintaining compliance with regulations, and ensuring professional accountability.

• Deliver personalized nursing care and monitor patient status
• Update and implement care plans
• Report changes and collaborate with healthcare teams
• Educate patients and families on health management
• Maintain compliance with safety and regulatory standards

Qualifications

• Active, unencumbered RN license in the practice state
• Current CPR certification
• TB test within the last 12 months
• Six months of hands-on nursing experience preferred
• Reliable transportation
