In-Home Healthcare LVN - Day Shifts

About the role

We are seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care in home settings, under the supervision of a Registered Nurse.

Responsibilities

• Deliver and coordinate patient care per physician orders
• Assess patient conditions
• Document activities and patient responses
• Update care plans
• Educate patients and families on disease management and self-care
• Maintain confidentiality
• Adhere to safety and infection control standards
• Ensure compliance with regulations and policies

Requirements

• Active, unrestricted LPN/LVN license
• CPR certification
• Recent clinical experience
• Pediatric experience preferred
