In Home Healthcare LVN - Day Shifts

We are seeking a Licensed Practical / Vocational Nurse (LPN/LVN) to provide skilled nursing care in a home setting. Key Responsibilities Include Delivering and documenting patient care according to physician orders and care plans.Assessing patient condition, recognizing changes, and reporting to supervisors or physicians.Implementing nursing actions to achieve desired outcomes and prevent risks.Educating patients and families on disease management, self-care, and prevention.Maintaining documentation, adhering to infection control standards, and complying with regulations.Participating in ongoing education and skill development. Qualifications And Skills Active, unencumbered LPN/LVN license in the practicing state.Current CPR certification and TB test within the last 12 months.Preferred six months of hands-on nursing experience; experience in pediatrics or home health is advantageous.Strong attention to detail, organization, communication, and problem-solving skills.Reliable transportation and physical ability to lift 50 pounds, reposition patients, and perform necessary movements. This role requires adaptability to various environments, physical stamina, and the ability to respond effectively in emergencies. The nurse will work under supervision, following state and federal regulations, and uphold confidentiality and professional standards.
