Responsibilities
- Engage with patients in their homes for health evaluations
- Conduct thorough medication and medical history reviews
- Perform physical evaluations and necessary diagnostic tests
- Travel locally within designated coverage areas
- Assess patient environments and report findings for comprehensive care
- Conduct virtual health visits when required
Qualifications
- Must hold an active, unrestricted medical license in the coverage area(s)
- Multi-state medical licenses are preferred
- Must be able to provide in-home evaluations and facilitate connections with healthcare resources
- Comfortable working in varied home environments, including those with pets or allergens
- Experience in patient-centered care
