About the role
FAMILY TRANSITION, INC. is a private for profit entity. Our philosophy is based on the family unit and its importance in society to provide for the well being of individuals. Our primary purpose is to identify at-risk youths, adults and families and provide quality services, which are designed to strengthen and preserve families and to maintain individuals at their home and in their community. In doing so, we strive to preserve and reunite the core family, assist families to develop appropriate boundaries while decreasing inappropriate behaviors, intervene in the school setting where needed and provide ongoing structure and support to the entire family. We strive to empower families to effectively utilize community and internal resources to achieve positive outcomes.
Responsibilities and Duties
- Provide mental health treatment to individuals receiving services (services may include Intensive In-Home, through individual and family therapy, behavior management, care coordination, crisis intervention, psycho-education, and skills training)
- Attend staff meetings and trainings as required (supervision is provided weekly and trainings are at least monthly)
- Collaborate with referral source and other treatment providers
- Create individualized service plans (ISPs) and implement the goals and objectives
- Complete required clinical documentation
- Ensure compliance with regulations set by governing bodies and maintain standards of care according to Family Transitions, Inc. policies and procedures
- Complete daily progress notes at minimum on a weekly basis by 9:00AM on Monday morning
Qualifications
- Qualified Mental Health Professionals-C (QMHP-C’s): Must be registered with the Board of Health Professions as a Qualified Mental Health Professional for Children. If not registered, must have at least a Bachelors degree in a human service field and 1 year of full-time experience.
- Residents in Counseling: Must have a Masters degree in Counseling with 500 internship hours or 1 year of experience.
- Supervisees in Social Work: Must have a Masters degree in Social Work with 500 internship hours or 1 year of experience.
Pay
Compensation is competitive and based on experience. Currently offering $23.00 - $27.00 per hour.
