About the Role
Provide one-on-one support to adults living independently or with caregivers, helping them succeed in their home and community environments.
Responsibilities
- Deliver personalized assistance based on individual plans
- Support daily activities such as personal care, household chores, recreation, transportation, and community integration
- Maintain accurate records
- Assist with health management and medication administration
- Accompany individuals to medical appointments
- Foster professional relationships with individuals, families, and case managers
- Ensure safety and confidentiality
Requirements
- High school diploma or equivalent preferred
- Six months of human services experience preferred
- Valid driver’s license
- Basic computer skills
- Ability to perform physical tasks involving lifting up to 100 pounds
- Must pass background checks, drug testing, and required training
