In-Home and Community Based Therapist

Responsibilities

• Work in-home and in the community
• Work collaboratively with the participating providers
• Provide crisis intervention
• Maintain a consumer clinical record (electronically) that accurately reflects all services provided on a timely basis and meets Agency and funding source standards
• Monitor and oversee service linkage in the mental health and non-mental health, health system
• Cook up and integrate services from multiple providers
• Monitor and oversee the provision of community-based services for consumers enrolled in the PACT program
• Provide clinical insight and treatment working with the PACT team

Qualifications

• Master's Degree in a behavioral health science or counseling specialty from an accredited institution and have two years of post-bachelor’s experience working with individuals with severe and persistent mental illness
• Must be willing to work in-home and in-community based settings
• Valid Driver's License required
