IHS Case Manager (502)

About the role

Lifeworks is a nonprofit human service agency that provides adults with developmental disabilities with the opportunity to live engaged and meaningful lives. We are seeking a dedicated IHS Case Manager to support individuals who live independently in the community.

Responsibilities

• Provide care and assistance to individuals who live independently in the community.
• Aid individuals with tasks such as banking, grocery shopping, budgeting, paying bills, planning vacations, managing healthcare, and coordinating and participating in a variety of one-on-one and group social activities.
• Promote optimal health for individuals through routine, preventive, and emergency health care.
• Ensure residential environments are safe, secure, and where individuals are free from harm.
• Offer education, training, counseling, and physical assistance when necessary to help individuals achieve Individual Service Plans (ISP) goals in areas like functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication skills, community mobility, self-esteem, and money management.
• Develop, write, and present annual ISP’s and semi-annual assessments in collaboration with other residential staff and the Director.
• Provide transportation to individuals as needed.
• Communicate with persons significant to the individual served (family, friends, guardians, funding sources, advocates, volunteers, employers) to ensure consistency of programming and support.
• Perform case management duties including maintaining individual confidential files, reviewing cases, documenting activities, and providing training.
• Attend regular meetings as necessary.
• Perform other duties and/or assignments as needed.

Requirements

• Experience working with Individuals with ID/DD
• Bachelor's degree from four-year college or university; related experience preferred
• Must pass Fingerprint background check, CORI, DPPC, and Driving background checks
• Valid Driver's License required
• Excellent verbal and written communication skills
