Job Duties
- Collaborate with IDD Crisis Services Team Leader to monitor crisis support services, including crisis responders.
- Utilize person-centered planning techniques to facilitate prevention and stabilization of crisis situations for clients with intellectual and developmental disabilities.
- Involves the family, guardian, and providers in planning by reviewing and coordinating the plan of care with them when appropriate and with the client’s consent.
- Identify and implement appropriate long-term supports and services, including residential placement for IDD individuals as needed.
- Collaborate with IDD Crisis Services Team Leader to develop a mobile crisis response system for IDD individuals.
- Collaborate with IDD Crisis Services Team Leader to develop a mobile crisis response system for IDD individuals.
- Collaborate with IDD Crisis Team to provide relevant staff training relating to crisis prevention and response, person-centered planning, and positive behavioral supports within the inter-agency training coalition.
- Provide outreach to community partners to educate and to enhance crisis prevention and response services within the region.
- Ensure availability for crisis intervention/emergency referrals, as well as scheduled on-call hours.
- Maintain HIPAA compliance and appropriate client confidentiality.
Qualifications
- A bachelor's degree in an acceptable human services or behavioral health field of study from an accredited school is required.
- At least one year of experience working in community-based intellectual and developmental disabilities (IDD) services.
- Excellent written and verbal communication skills.
- A valid driver's license and the ability to operate a motor vehicle.
