About the Role

The professional nurse uses the nursing process to deliver patient- and family-centered care, valuing the uniqueness of each individual and addressing the physical, psychological, emotional, and social needs of the diverse patient population served by Jefferson Health.

Develops individualized nursing plans of care in partnership with patients and their designated care partner(s), ensuring alignment with cultural beliefs and values.

Responsibilities

  • Integrates Evidence-Based Practice, clinical expertise, and patient/family preferences and cultural beliefs to deliver optimal healthcare.
  • Partners with patients and their family/designated care partner(s) to identify learning needs and provides education in accessible, affirming ways.
  • Minimizes risk of harm by adhering to established nursing practices, protocols, and individual performance standards.
  • Uses information technology to communicate, manage knowledge, mitigate error, and support ethical decision-making.
  • Monitors care outcomes using data and implements improvement methods to enhance quality and safety.
  • Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making.
  • Provides safe, compassionate, and coordinated care based on respect for patient preferences, priorities, needs, and cultural values.
  • Creates an inclusive environment, demonstrating problem-solving and conflict-resolution skills.
  • Commits to continuous professional growth, acts as a primary preceptor for appropriate staff, and participates in hospital and practice-based committees, including safety and quality improvement.
  • Demonstrates willingness to give and receive feedback, integrating changes into practice.

Requirements

  • Education (Required): Graduate of an accredited school of nursing.
  • Education (Preferred): Bachelor’s Degree (BSN) or completion of BSN within 3 years of hire strongly preferred.
  • Experience (Preferred): Previous experience as an RN in an acute care setting strongly preferred.

  • Patient- and Family-Centered Practice: Ability to convey respect and collaborate with patients, families, and designated care partners in care planning.
  • Cultural Awareness: Knowledge of diverse backgrounds and ability to connect effectively with respect.
  • Effective Communication: Ability to transmit, receive, and interpret ideas, information, and needs through appropriate communication behaviors, using partnership language with patients and families.
  • Patient Assessment and Care: Ability to assess ailments and injuries and implement treatment plans.
  • Patient and Designated Care Partner Education: Knowledge of child and adult education techniques to educate on conditions, treatments, medications, and recovery steps.
  • Medication Administration and Prescription: Knowledge of medications and ability to administer and prescribe for continuing treatment.
  • Discharge Planning: Ability to partner with patients and families to develop discharge plans and arrange follow-up care.
  • Assessment and Diagnosis: Knowledge of tools and techniques for patient assessment, nursing diagnosis, and clinical judgment.
  • Health Information Documentation: Ability to record medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, and test results accurately.
  • Delegation: Ability to appropriately and respectfully delegate tasks to other team members.

Qualifications

  • Licenses and Certifications (Required):
    • BLS & ACLS – American Heart Association
    • RN – Licensed Registered Nurse (PA – State of Pennsylvania)
    • Completion of Dysthymia Course from American Association of Critical Care Nurses
  • Experience: ICU & Stepdown experience.

Workday Day shift.
