Its services for individuals with mental health conditions provide daily living support and community-based programs that teach valuable life skills through instruction and real-world experiences.

Responsibilities

  • Coordinate and manage the care of consumers in Integrated Case Management Services (ICMS).
  • Provide ongoing support to consumers to help them function independently and access mental health services.
  • Conduct home visits to engage with consumers in their environments to reduce risks of hospitalization and promote functional integration into the community.
  • Afford consumers with needed services and ensure transportation to appointments.
  • Develop individualized service plans and ensure the consumer’s engagement with community mental health and non-mental health services.
  • Assess the need for crisis intervention and assist in psychiatric emergency situations.
  • Encourage wellness and recovery while maintaining individual dignity and respect.
  • Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.

Requirements

  • Education & Experience:
    • Master's degree in social work, counseling, psychology or related field, from an accredited institution and possess one (1) year of relevant supervisory experience;
    • OR Bachelor of Arts in social work, counseling, psychology or related field, from an accredited institution, and three (3) years of post-graduate work experience in mental health and possess one (1) year of relevant supervisory experience.
  • Skills: Skilled in budget management, proficient with technology including Electronic Health Records, and equipped with strong verbal and written communication abilities.
  • Attributes: Able to manage multiple priorities in a fast-paced, deadline-driven environment while maintaining a compassionate, flexible, and client-focused approach to care. This role requires driving as an essential function. Candidates must have a valid driver's license and be subject to post-offer verification of driving eligibility consistent with company insurance requirements.

  • Compensation: Bachelor’s Degree: $44,000 – $48,000 annuallyMaster’s Degree: $48,000 – $52,000 annuallyFinal salary is based on education, experience, and internal equity.
