Hospice RN Case Manager (Middlesex / Somerset County, NJ)

About the Role
Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to service patients at their homes or medical facilities in Middlesex/Somerset County, NJ. As a member of the interdisciplinary team, the RN Case Manager works under the direction of the Director of Nursing, the attending physician, and the interdisciplinary group to maximize patient comfort and health through skilled nursing assessments, planning, and care for both patients and their loved ones or caregivers.
Responsibilities
- Assume patient caseload assigned by the Patient Care Coordinator or designee.
- Manage a patient caseload, including assessing, planning, implementing, and evaluating phases of the nursing process.
- Initiate communication with attending physicians, Hospice staff, and other agencies to coordinate optimal care and resource use.
- Maintain regular communication with the attending physician regarding patient and caregiver needs.
- Seek input from other team members to enhance the patient plan of care.
- Provide accurate and timely documentation in electronic medical records.
- Participate in and contribute to Interdisciplinary Group meetings.
- Coordinate physical care by teaching caregivers, volunteers, or providing direct care as needed.
- Meet regularly with Hospice nursing staff to review caseload issues and share professional support.
- Attend patient care conferences and facilitate discussions on caseload issues.
- Supervise and document the work of Nursing Assistants (NAC) every two weeks or as assigned.
- Inform the Patient Care Coordinator of unusual or problematic patient/family issues.
- Serve as an associate nurse for other primary nurses during on-call time or as requested.
- Provide support at the time of death and during the bereavement period.
- Participate in Hospice orientation and in-service training programs.
- Engage in agency and community programs to promote professional growth and hospice care understanding.
- Maintain familiarity with agency policies and state/federal regulations governing hospice care.
- Accept additional assignments as appropriate.
Requirements
- Graduate from an accredited School of Nursing.
- Licensed in NJ as a Registered Nurse by the New Jersey State Board of Nursing.
- One year of RN experience; hospice experience preferred.
- Excellent assessment and problem-solving skills.
- Strong organizational skills and knowledge of time management and case management processes.
- Licensed driver with an insured, reliable automobile.
- Strong verbal and written communication skills.
