Community Based ICMS Case Manager - South Jersey

About the role

The Case Manager works in an adult case management setting serving adults with severe and persistent mental illness. Goals include reducing hospitalization frequency and length, enhancing community resource connections, and improving quality of life.

Responsibilities

• Provide intensive community-based services to seriously and persistently mentally ill consumers
• Facilitate service linkage across mental health and non-mental health systems, coordinating and integrating services from multiple providers
• Support consumers directly through activities such as transportation, treatment team coordination, application filing, advocacy, escorting to appointments, daily living skill remediation, and money management
• Maintain accurate electronic clinical records reflecting all services provided in a timely manner and meeting agency and funding source standards
• Complete daily activities in the Electronic Clinical Record
• Provide 24/7 on-call crisis intervention on a rotating basis, including evenings and weekends
• Work collaboratively with the Clinical Team
• Drive and travel within Southern New Jersey

Requirements

• Masters of Social Work (MSW) or a Master’s (MA/MS) degree with clinical training OR;
• Bachelor’s (BA/BS) degree in Psychology, Social Work, Mental Health Counseling plus (1) year relevant experience
• Valid Driver's License in good standing
