Responsibilities
- Interject objectivity, maximize efficiency and work collaboratively with patient, physician, family and in the development and execution of a healthcare plan.
- Assist client’s self-direct care and make decisions to the degree possible.
- Appraise the need for intervention by gathering relevant data and conducting an objective evaluation.
- Evaluate triggers to identify potential clients suitable for effective case management intervention.
- Promote effective communications between all team members, client, family, provider and other parties involved in the case.
- Ensure the clients best interests are represented in the evaluation, treatment and management of their health care needs.
- Develop a healthcare plan that addresses the immediate, short-term and ongoing needs as well as where and how client’s care needs to be met.
- Routinely make comprehensive and independent assessment of the client’s status and progress toward reaching the goals set forth in the care plan.
- Perform other duties as assigned.
Qualifications
- Education: Bachelor's Degree in Nursing (BSN) is required.
- Experience: Minimum two (2) years of experience in a health related setting or Community Based Organization.
- Knowledge and Skills: Excellent relationship building skills, excellent and effective written/verbal communication skills, ability to effect change, critical analysis, excellent planning and organizing skills, ability to promote client/family autonomy. Knowledge of funding resources, services, and clinical standards and outcomes, ability to fully utilize BHMC’s electronic record systems, equipment and other healthcare and billing systems relevant to this position.
- Licenses, Certifications: BLS Certification is required.
Physical Requirements
- Position requires prolonged periods of standing, reaching, walking throughout the working day.
- Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
