ICM Case Manager (26-74)

About the role

The Case Manager will be responsible for Person-Centered Planning to develop an Individual Plan of Service. They will provide on-going linking, coordinating, and monitoring of services.

Responsibilities

• Develop and implement Person-Centered Plans
• Coordinate and monitor services for individuals with mental illness, substance abuse disorders, or intellectual or developmental disabilities
• Work within a hybrid schedule, combining office work at 790 Fuller Ave NE Grand Rapids, Michigan 49503 and community-based work
• Perform job duties from 8:00 am to 5:00 pm, Monday through Friday

Requirements

• Bachelor’s degree in human services related field
• Six months of full-time experience working with persons with mental illness, substance abuse disorder, or intellectual or developmental disabilities
• Valid driver’s license and access to reliable transportation
• Strong organizational skills and experience in person-centered planning
• Demonstrated understanding of persons with severe mental illness and substance use disorders
• Demonstrated effective communication and active listening skills

Qualifications

• Licensed BSW preferred
• Knowledge of trauma-informed theories, principles, and practices
• Lived experiences with mental illness/developmental disabilities/substance use disorders valued

Pay

Final job offer is based on internal equity, skills, education, experience, and credentials. Compensation range is $21.99 - $29.15 per hour.

Schedule

This position is 40 hours a week, Monday through Friday, 8:00 am to 5:00 pm. There are no weekends, on-call, or holiday requirements for this position. The employee works a hybrid schedule, combining office work at 790 Fuller Ave NE Grand Rapids, Michigan 49503 and community-based work.
